Questions:

• Do you experience distorted thinking patterns (Polarized Thinking, Overgeneralization, Catastrophizing, Personalization, Mind Reading, Mental Filtering, Discounting the Positive, “Should” Statements, Emotional Reasoning and Labeling)?
• What stressors do you focus on or think about often (Ex. social media, bills, debt, world news, failure, work, school, traffic, cleaning, pain, bad health, childhood trauma, past mistakes, disappointment, personal appearance, self-criticism, self-doubt, etc.)?

 Questions:

• How has Adverse Childhood Experiences affected your way of think and act (Ex. physical abuse, Sexual abuse, Emotional abuse, Physical neglect, Emotional neglect, Mental illness, Divorce, Substance abuse, Witness mother being abused, or having a relative who has been sent to jail or prison)?
• What high stress negative emotions do we often experience or feel (Ex. Depression, Anxiety, Fear, Doubt, Shame, Insignificant, Unheard,  Disappointment, Worry, Loneliness, Confusion and Embarrassment)? 

Questions:

• What negative habits do you have that impact your physical health (Ex. smoking, drinking, drug use, emotional or binge eating, lack of or too much sleep, sedentary lifestyle, etc.? 
• What false or limiting beliefs keep you from reaching your wellness goals (Ex. I'm sick, I'm in pain, I don't like to eat healthy, I don't like to workout, I have no time, I'm so tired, I will always be overweight and I do have an addiction)? 

Questions:

• Are your relationships toxic or healthy (Ex. domestic violence substance or alcohol abuse use, infidelity, narcissistic behaviors, etc.)? 

• Do you feel appreciated, respected and heard in your relationships (Ex. others treat you unfair, unrealistic expectations by others, no boundaries, fear commitment, fear abandonment, fear rejection, not feeling good enough, low self-esteem and are you an abuse victim)?
